2017-2018 Electronic Enrollment Forms

Thank you for choosing the Williamstown Independent School District to educate and impact your child's future! We are one of the oldest operated schools in Kentucky. Since 1890, we've provided our students with a rich tradition of excellence and we are proud to be known as "One of the best reasons to live in Williamstown."


 Please call the Welcome Center at 859-824-4184 to schedule an appointment so you can complete the paperwork necessary to get your child registered for school.  To save time, you may download and complete the following forms located in our District Forms section.


Are you New to Kentucky Schools?


Below is a list of documents you will need to enroll your child in school if they are being enrolled in a Kentucky school for the first time.

  • Legal Birth Certificate
  • Kentucky Certificate of Immunization
  • Proof of a Preventative Health Care Examination, documented on the appropriate Kentucky form
  • Proof of an eye examination by an optometrist or ophthalmologist, documented on the appropriate Kentucky form
  • Proof of a Dental Screening or examination (for 5 or 6 year olds), documented on the appropriate Kentucky form

You will also need:

  • Proof of Custody/Guardianship (if applicable)
  • 2 Proofs of Residency (ex: rental/lease agreement, purchase contract, utility bill)
  • Previous School Information (Name, Address, Phone and Fax Number)

Are you transferring from a Kentucky school?

You will need the following documents to transfer your child from a Kentucky school to a school in our district.

  • Proof of Custody /Guardianship (if applicable)
  • 2 Proofs of Residency (ex: rental/lease agreement, purchase contract, utility bill)
  • Previous School Information (Name, Address, Phone and Fax Number)

All Out of District Students applying for enrollment in our school district will need to complete and submit an Out of District Application & Request for Records for Consideration as an Out of District Student before beginning the enrollment process. Applications & Request forms must be picked up from the  Board of Education office.

Superintendent's Message

Dear Williamstown Independent Families,

Fall has arrived and the annual Fall Festival has concluded. It was a wonderful event, with beautiful weather.  Thank you to all who helped with the planning and execution of the event.  Fall also means Fall Break is just around the corner.  Schools will be closed October13-17 for students, but teachers will return on the 17th for professional development so they can continue learning and growing to best serve our students.

Cooler temperatures are outside, but inside our schools, it is hot!  Staff is busy ensuring a positive and safe learning environment and students are busy with various activities including both academic and athletic events.  Speaking of athletics, the new high school soccer team has won a tournament and many games against tough completion.  The cross-country team is on fire, winning several tournaments and the NCKC championship.  The future of volleyball looks bright as well.  We are so proud of our student athletes.  They shine in the classroom and on the field.

In terms of academics, we received results from last year’s K-PREP scores.  Since the Kentucky Department of Education is going through a transition period with the accountability system, schools and districts were not ranked as they have been in the past.  Schools received individual students’ scores and overall scores in achievement, gap, and growth.  I am excited to announce that our overall achievement scores were up in all three schools.  Our gap scores were also up, and in some cases, great gains were made in this area.  Although our scores in achievement were up, our growth scores decreased a bit. This is perplexing to us, but the way in which growth is calculated will be different from the previous system.

Principals, administrators, and teachers have met to review student data and overall scores.  Together, we are working on varied instructional strategies, curriculum design, and other ideas to ensure student success.  Our district is fortunate to have a great staff.   I encourage you to share with me in valuing, respecting, and supporting our teachers because great teaching is the key to student learning.

It is definitely an exciting time to be a Demon.  Williamstown IS a great school system, filled with the best students and individuals who care for those students and desire to prepare them well for whatever lies ahead in their future.  I consider myself fortunate to live in Williamstown, as a member of a community that cares deeply about education, and to lead a district filled with students and staff that I love.

I am very proud of the accomplishments of our students and staff thus far, and look forward to everyone working together to create many more.
